Affidavit of Health and geolocation Authorization - A medical electronic sworn statement and commitment to isolate or quarantine a traveler, must be completed by anyone arriving in Peru. It can be found on the website of the National Office for Migration: https: //e-notificacion.migraciones.gob.pe/dj-salud/ and must be completed no earlier than 72 hours before departure.
